HCFZ draws its community health project focus areas from fifteen (15) health program categories informed by Sustainable Development Goals (SDGs) and local community’s health needs. Each program focus area gives rise to more than one community health projects. Here below are the fifteen (15) health program focus areas under which potential project support partners can collaborate with HCFZ.

Here below are the Disease prevention and Control program focus areas under which potential project support partners can collaborate with HCFZ:
a) Living a healthy lifestyle promotion: Physical activity, diet and nutrition, no smoking campaign, no harmful use of alcohol, etc. |
b) Outreach activities: Health education and screening for cardio-vascular diseases, heart diseases, high blood pressure, high cholesterol, diabetes, cancers, eyes and vision, oral health, osteoporosis, bone density and weight, body and index, including vaccinations. |
c) Vaccinations Campaign: Vaccination campaign against all World Health Organisation (WHO) recommended diseases like COVID-19, malaria, cervical cancer, diphtheria, Ebola, Hepatitis B, polio, measles, Influenza, rabies, tetanus, yellow fever and typhoid |

HCFZ’s intervention in community health needs is done in the ascending order beginning with individual health needs, followed by family health needs, then community health needs, up to national health needs. Where consented efforts are needed to resolve international health needs, the entry point of HCFZ is the region/continent, then finally global participation as as the case may be (see the drawing below).
There are established community committees known as Community Projects Implementation and Management Committees (CPIMC) of HCFZ that implements community health projects on the day-to-day basis, to the benefit of individuals, families and communities under their respective jurisdiction. Each CPIMC reports to the respective HCFZ Provincial Projects Monitoring and Evaluation Committee (PPMEC) for technical support. PPMEC reports to the Board of Trustees for resource mobilisation, coordination, and policy guidance.
